Moving into Kerikeri Retirement Village’s Care Facility.

1. Assessment by the Northern District Health Board’s Aged Care Needs Assessment and Service Coordination (NASC) service is a requirement of entry.  The NASC provides assessment to identify your level of need for ongoing health care and support.  Depending on the outcome of the assessment, the assessor may authorise entry into residential aged care.  Visit northlanddhb.org.nz for more information on the NASC service or talk to your GP.

2. Contact… to check availability and arrange a personal meeting with the Clinical Manager.  At this meeting the Clinical Manager will talk with you about your preferences, provide an admission pack and answer any questions you may have.  The meeting may also be followed by a tour of the property. 

 

3. Return your completed admission pack to the Clinical Manager.  There are number of forms for completion in the admission pack and it may be helpful to have a family member or friend assist you.  Should you have any questions regarding the contents of the admission pack, please contact for assistance.

4. Move in.  Moving from your current home will require some planning and preparation.  Allow yourself time for this transition.  Our staff and residents will have a lot of information and advice to share about local services that can assist you with the practicalities of moving.  Please ask us.
